This case study from Penn Medicine describes an 8-week pilot to validate a scalable heart failure (HF) remote monitoring workflow using implantable device diagnostics, especially Boston Scientific HeartLogic and CardioMEMS. The program was built in response to rising HF hospitalizations, readmissions, and costs, and aligned with Heart Rhythm Society guidance supporting customized alerts and remote monitoring of HF diagnostics.<br /><br />At baseline, Penn Presbyterian Medical Center had strong device-monitoring infrastructure but only 18 patients enrolled in a defined HF protocol, despite more than 5,500 device patients being monitored. The main challenge was that actionable HF signals were visible inside EP-centric workflows, but there was no standardized process for symptom screening, escalation, or follow-up.<br /><br />The pilot introduced a structured workflow with clear thresholds, outreach scripts, documentation templates, and a diuretic protocol. During the 8-week pilot, 88 patients were monitored in the HeartLogic cohort, 78 alerts were identified, and 72 of 78 alerts (92%) received outreach within three days. The average time spent per actionable alert was 12.5 minutes, and total RN/APP time was 16 hours and 10 minutes. The workflow expanded from 18 baseline CardioMEMS patients to 154 active HF monitoring patients without adding staff.<br /><br />The model scaled through shared EP-HF ownership, AI-assisted triage, certified device specialist review, and regular multidisciplinary review. Financially, the pilot suggested a Medicare reimbursement opportunity of $10,590 during the 8-week period tied to 154 routine HF transmissions.<br /><br />Overall, the presentation shows that device-derived HF diagnostics can be transformed into proactive, protocol-driven care with earlier intervention, improved workflow efficiency, and a foundation for sustainable scaling.
