This document explains a 3-step Epic Reporting Workbench process for creating the MedAxiom CPT Count with Modifiers submission. The submission is a count of CPT codes by rendering/billing provider, modifier, place of service type, and location groups from both Hospital Billing and Professional Billing.

Step 1 is to create and update a provider list file. Save a CSV named <strong>MedAxiom_CPT_Mod_Provider.csv</strong> in the bulk insert directory <strong>\\i-vClarityProd\BulkInsert\Finance\MedAxiom\CptModifier</strong>. The file should include only <strong>Provider Name</strong> and <strong>NPI</strong> in that order, with no extra rows, columns, spaces, special characters, or formulas. Provider ID is preferred over NPI if possible. Send the file to J.L. Brown or submit an IT ticket so Enterprise Analytics can place it in the directory.

Step 2 is to run <strong>MedAxiom CPT Count with Modifiers 1-Location (SQL)</strong> to generate a distinct list of Epic locations. Save the output as <strong>MedAxiom_CPT_Mod_Location.csv</strong> in the same bulk insert directory. Add or update the <strong>LocationGroup</strong> column, using <strong>“Exclude”</strong> for locations that should not be included in the submission. Required columns are <strong>LocationID</strong>, <strong>Location</strong>, and <strong>LocationGroup</strong>. Then send the file to J.L. Brown or Enterprise Analytics for loading.

Step 3 is to run <strong>MedAxiom CPT Count with Modifiers 2-Hospital Account CPT Detail (SQL)</strong> to generate detailed CPT/modifier data for validation. The tip sheet recommends running separate versions for PB and HB transactions if the result set is large.

Step 4 is to run <strong>MedAxiom CPT Count with Modifiers 3-Submission (SQL)</strong> to produce the final submission file and submit the results.
