Accurate evaluation and management (E/M) coding starts with documentation that clearly reflects the complexity of cardiovascular care. Through practical cardiovascular scenarios, this session will examine the documentation behind medical decision-making (MDM), E/M level selection, G2211, modifier 25, time-based services, same-day procedures and diagnostic testing.

Participants will work through common areas of physician-coder confusion, including chronic disease management, independent interpretation of test results, prescription drug management, decisions regarding procedures, and distinguishing a separately identifiable E/M service from work inherent in another service. Geared toward coders, auditors, compliance teams and revenue cycle teams, this webinar will connect documentation practices to coding accuracy, compliance, revenue integrity and physician workflow efficiency.
